Rules for Point
Card Usage

Membership
Qualification

All patrons of IZAKAYA ICHIBAN
Malaysia can apply for this Point Card.

Kindly fill in the Membership
Application form and submit to the restaurant counter staff.

The card will be issued under the
name of the applicant. Usage is restricted to the applicant and
his/her family members only.

Point Usage and
Privileges
-
This card is only valid in
IZAKAYA ICHIBAN Malaysia.

1 point is awarded for every RM20
spent at our restaurant (service and tax amounts not included;
fractions are rounded down).

After accumulated 30 points,
value of RM30 voucher will be given. After accumulated 50
points, value of RM50 voucher will be given. After accumulated
100 points, value of RM100 voucher or 1 bottle of Shochu (Hakata
Kojyoro, value of RM130) will be given.

Voucher is not refundable and
balance change is also not refundable when use of voucher.
Voucher without Authorized signature and date is invalid. The
term of validity is 3 months from the issued date.

Please inform our staff if you
have forgotten to bring along your card. In such case, we will
put a stamp on your receipt for point crediting purposes on your
next visit.

This card can be used with other
vouchers. However, points will only be given for the actual
amount paid in cash/credit card.

Points accumulated are not
transferable and cannot be exchanged for cash.

Loss and Re-issue of
Card

Kindly inform our staff if your
card has been misplaced or stolen.

Points accumulated in the lost
card will be forfeited. The restaurant shall not be held
responsible for points forfeited.

Please fill up Membership
Application Form should you wish to re-apply for membership. A
fresh card will be issued.

Change of Particulars

Please inform our
staff when there is a change in your particulars, eg. Address,
Contact, etc.

Withdrawal and
Termination of Membership
-
Kindly inform and return the card
to the counter staff should you wish to withdraw your
membership. Remaining points on the card will be forfeited.

Your membership will be
terminated if you violate any of the rules stated above or
should there be any misuse of points. Remaining points on the
card will be forfeited. The card has to be returned to the
restaurant.

DM, Email, Pamphlets,
Printed Materials, etc.

As a privileged
customer, the restaurant shall, from time to time, send you
promotional materials via various marketing channels.

Amendment to
Regulations
-
The Restaurant
Management reserves the right to amend the membership
regulations, to stop or end any service without prior
notification.
